Letter of Intent varies somewhat according to what your state requires you include in terms of info. I notice you're in Delaware, however, and the following gives detailed info on what you have to do / send in:
Delaware Home Education Association: Getting Started _________________ The Farming Game - Software Edition for PC, Mac, Linux

| i started by going down to the schools office. they directed me to the school super intendant. she was extremely helpful. Even though i was already 3 days past the deadline for filingit she said they would just say it had gotten misplaced and file it anyway. |
