First Time Homeschooling

Have an 8th grader who was attending school but since the second week of January has not because of her high anxiety. She rather be homeschool and I don't have a problem with that but where do I start? She is an honor student and don't want her to miss out on finishing 8th grade this year. Is there a form I have to sign? Do I let the school board know? Where do I find these forms? I'm in New Jersey. I use to know people who homeschooled but lost touch. Does anyone have some advise? Please help ASAP
