I printed the form out from my state's website which is Georgia. It asks for the 12 month period for the home study program. My daughter started homeschool on October 9th and before that she was in public. So what dates do I put down, Oct. 9 2006- Oct. 9 2007 or the date when she started the year with public school?

If your daughter was pulled out in the middle of the school year, you should start your year at the beginning of the school year, and make a note on the form that x days of your current year were covered at x school.
