Report Card for Minnesota

Hello. I have been lurking here reading everything that I can, but have not actually posted anything yet. I have a question that I have not been able to find the answer to yet, so I hope that you can help. I live in Minnesota, and do not have a bachelor degree, so I have to submit a report card this month. I have a 6th and a 2nd grader, and I am not quite sure how the report card should look, what it should and should not include. Any advise would be greatly appreciated. Thanks,Stacy
