There may be placement tests offered by your state, but if you're going to be using homeschool curriculum, the best idea is to use the placement tests offered by whatever curriculum providers you decide to go with, since grade level is largely subjective:
Saxon Math
Alpha Omega
Switched-On Schoolhouse
Calvert
etc.
There are also numerous books and online resources for placement testing, or you can just borrow some books for the grade range you think your daughter is in and put together a test from a sampling of problems from each grade.

This is why we use Charlotte Mason Method
I can adjust curriculum to her level=she is twelve but all over the board academically from grade 2 math to fifth grade reading/spelling!
Flexibility is key. A bough curriculum for a child who is really varied in academic levels really doesn't work well.
I make my own curriculum based on this method.
